I am a plant healthcare specialist for a large tree service in Pa with years of schooling in this. Right now in PA this is a big problem on all pears. Unfortunately as a homeowner your options are limited. Rust is just a fungus so your best bet would be let it run its course and when all the leave have dropped in the fall make sure to clear them all out from under the tree to prevent new spore from forming and reinfection the tree next year. your other option if you would to try it is to use Bayer Fungus Fighter Concentrate or Bayer Fungus Fighter Plus. They are the only two fungicides on the market for homeowner use, I can't tell you which works better or if either do at all since I've been using commercial grade materials for years.
