Misplaced jury duty notice

I received a notice for jury duty.  I put it in my purse.  Tonight at a family party it was in my purse, but when I got home it was gone.  I don't know at what point it may have fallen out.

Who do I contact? I don't want to get in trouble for not responding, but without the notice I don't know any of the info they need.
